Health professionals from the fields of health care, teaching, research and management have collaborated in this book to contextualise and address different aspects of primary care and community health.
The publication is structured in nine chapters and sixty-three sections, which describe the Catalan health model, health and its determinants, community health, primary care, the primary care nurse and community health, care for people throughout their lives, care for people with health problems, care for complexity and end of life, and research in primary care and community health. Each chapter incorporates the knowledge on the topic according to the evidence and professional experience of its authors.
